Brief Rapid Communication Myocardial Catecholamine Content After Heart Transplantation

چکیده
Myocardial catecholamine levels have not yet been determined in the transplanted human heart. We measured norepinephrine, epinephrine, and dopamine in endomyocardial biopsies from 19 short-term (organ age, 6.6±+6 months) and five long-term (organ age, 62±2 months) heart transplant patients. Results were compared with those from 10 normal control subjects. In 17 of 19 short-term heart transplant patients, myocardial catecholamines were undetectable, indicating values below 0.1 pg/,ug noncollagen protein, which was the detection threshold ofour assay. In the remaining two patients, myocardial catecholamines (pg/pg noncollagen protein) were norepinephrine (1.4 and 3.2), epinephrine (0.8 and 1.9), and dopamine (0.9 and 2.3), respectively. In the five long-term heart transplant patients, myocardial catecholamines were not detected. Catecholamine concentrations in 10 healthy control subjects were norepinephrine (10.3 +2.9), epinephrine (0.36 ±0.51), and dopamine (0.52 ±0.40). Low myocardial norepinephrine levels (<20% of control values) with unexplained high levels of epinephrine and dopamine were found in single transplant patients. In most heart transplant patients, however, myocardial catecholamines were undetectable up to five years after transplantation, indicating that the adrenergic response of these hearts probably depends on variations in plasma catecholamines or cardiac 3-receptor density. Case Report : Pheochromocytoma - related catecholamine cardiomyopathy with successful outcomes after orthotopic heart transplantation
Pheochromocytomas (rare catecholamine-producing neuroendocrine tumors) have many different manifestations, and complications can occasionally include myocardial infarction and cardiomyopathy. In a majority of cases, cardiomyopathy reverses following medical or surgical treatment of the pheochromocytoma. We report a case of a 28-year-old male patient with preoperative diagnosis of pheochromocyto...
متن کاملAre Stem Cells the next Therapeutic Tool for Heart Repair?
Cardiovascular disease remains the leading cause of morbidity and mortality in the United States and Europe. In recent years, the understanding that regenerative processes exist at the level of the myocardium, has placed stem cell research at center stage in cardiology. A stem cell is a cell that has the ability to divide (self replicate) for indefinite periods often throughout the life of the ...
متن کاملMyocardial performance in hemorrhagic shock in the dog and primate.
Myocardial performance was investigated in nine dogs and six monkeys during experimental hemorrhagic shock under conditions of constant afterload. Left ventricular function curves were obtained by opening an aorta to left atrial bypass, and simultaneous recordings of stroke volume, peak stroke power, maximal dP/dt, maximal dF/dt, heart rate, atrial pressure and end-diastolic pressure were made....
متن کاملRapid reversal of heart failure in a patient with phaeochromocytoma and catecholamine-induced cardiomyopathy who was treated with captopril.
A patient with a phaeochromocytoma and severe left ventricular heart failure caused by a catecholamine-induced cardiomyopathy is described. The clinical signs of congestive heart failure resolved rapidly on treatment with captopril and myocardial performance became normal within two weeks of medical treatment with captopril for one week and with captopril in combination with phenoxybenzamine fo...
متن کاملReversible left ventricular dysfunction associated with raised troponin I after subarachnoid haemorrhage does not preclude successful heart transplantation.
Transient abnormalities in ECGs, echocardiograms, and cardiac enzymes have been described in the acute setting of subarachnoid haemorrhage. In addition, left ventricular dysfunction has been reported at the time of brain death. A patient with an acute subarachnoid haemorrhage who presented with raised troponin I (TnI) concentrations and diffuse left ventricular dysfunction is described. After d...
